Provide primary care services such as preventive screenings, chronic disease management, immunizations, and health education tailored to individual patient needs. Manage acute and chronic conditions in an outpatient primary care clinic. Utilize EMR system practice fusion to document medical records accurately and efficiently while maintaining HIPAA compliance. Collaborate with multidisciplinary teams on discharge planning, case management, and utilization review to optimize patient outcomes. Engage in telehealth consultations to extend care access and support patients remotely using secure virtual platforms. Experience Proven experience as a Family Nurse Practitioner with demonstrated proficiency in primary care delivery. Clinical experience with diverse populations including pediatrics, geriatrics, behavioral health, and individuals with disabilities or complex medical needs. Knowledge of CPT coding, ICD-10/ICD-9 coding standards for accurate documentation and billing processes.
