Job Description:
Aveanna Healthcare, a leader in home healthcare services, is
seeking a dedicated and compassionate In-Home Healthcare LVN with a
focus on high acuity patient care. This position offers a unique
opportunity to deliver personalized nursing care to patients in
their own homes, working with a range of cases including complex

experience in complex care scenarios. Provide high-quality nursing
care to patients with complex health conditions in a home setting.
Assess patient conditions during visits and perform continuous
assessments and evaluations. Administer medications, monitor vital
signs, and provide intricately detailed care plans tailored to each
patient’s individual needs. Work collaboratively with physicians,
therapists, and families to effectively manage patients' healthcare
plans. Educate patients and their families on health maintenance
and disease prevention. Maintain accurate patient records and
prepare clinical notes on all in-home visits. Assist patients with
day-to-day activities that improve their quality of life and foster
independence where possible. Strong clinical skills in a high
acuity healthcare setting Excellent communication and interpersonal
skills Ability to think critically and make decisions in
high-pressure situations Proficient in administering medications
and operating medical equipment A compassionate approach to patient
care Medical Insurance Dental Insurance Vision Insurance Life
Insurance 401k Current Licensed Vocational Nurse (LVN) licensure in
the state of practice CPR/BLS certification Valid driver’s license
and reliable transportation Experience with ventilator care,
tracheostomy, and feeding tube management preferred
